Getting Started
How to Start Home Care Services in NYC
Medicaid, MLTC plans, long-term care insurance and private pay all work with us. Our intake team guides your family from the first call to the first visit.
- Call or request an assessment. Reach our 24/7 line at (718) 390-0561. Tell us who needs care and how it will be paid.
- We check coverage. We confirm Medicaid, MLTC, waiver or insurance details. Then we explain any costs before care begins.
- Nurse assessment and care plan. A Registered Nurse plans care around medical needs, routines, culture and language.
- Care begins at home. We match a trained aide or nurse. After that, we supervise care and adjust the plan as needs change.
Frequently Asked Questions
Home Care Services in NYC: Your Questions Answered
What home care services does Fadmo Health provide?
Fadmo Health is a Licensed Home Care Services Agency (LHCSA) in New York City. We provide home health aides, personal care, skilled nursing and wound care. We also offer medication and IV administration, hospital sitters and NHTD and TBI waiver services.
Which areas of New York do you serve?

How do I get a home health aide through Medicaid in NYC?
Most adults receive Medicaid home care through a Managed Long Term Care (MLTC) plan. First, the person needs Medicaid. Next, the New York Independent Assessor (NYIA) completes an assessment. Then you enroll in an MLTC plan and choose an agency in its network. Fadmo Health works with VillageCareMAX, VNS Health, ArchCare and Anthem (formerly Empire/Amerigroup).
Can I switch to Fadmo Health if I already have a home care agency?
Yes. If your MLTC plan works with us, ask your care manager to move your home care to Fadmo Health. You can also call us, and we will help you start the request.
What is a hospital sitter and who pays for it?
A hospital sitter is a trained companion who stays with your loved one in the hospital or rehab. Sitters offer comfort, help prevent falls and keep the family informed. This service is usually private pay, since Medicaid does not typically cover it.
Do you provide skilled nursing and wound care at home?
Yes. Our Registered Nurses and LPNs provide skilled nursing at home. This includes wound care, medication management and IV administration, always following the physician's orders.
Do you offer 24-hour home care?
Our care line answers 24/7. In addition, we staff hourly, overnight and 24-hour schedules based on what the plan approves or the family arranges.
Why should I choose Fadmo for home care services in NYC?
Our nurses lead every care plan, and our caregivers are trained and supervised. Fadmo Health is also Joint Commission certified, bonded and insured. See what sets our agency apart.
