VillageCareMAX home care in the Bronx is available through the plan’s Medicaid Managed Long Term Care (MLTC) program. It serves eligible members who need help at home for more than 120 days. An MLTC plan is a Medicaid health plan that arranges long-term services like home health aides, personal care, and nursing. VillageCareMAX does not send aides itself. Instead, it works with licensed home care agencies, and FADMO Health and HomeCare Agency is one of them. Below, we explain who may qualify, how the process works, and how to ask for FADMO.

Who may qualify for VillageCareMAX home care in the Bronx?
VillageCareMAX lists four basic requirements for its MLTC plan. A person must be 18 or older and eligible for Medicaid. They must also be able to live safely at home when they enroll. Finally, they must be expected to need long-term care services for more than 120 days.
New York State also requires most adults age 21 and older who have both Medicare and Medicaid to join an MLTC plan if they need community-based long-term care for more than 120 days. What does VillageCareMAX cover at home?
According to VillageCareMAX, its MLTC plan covers home-based services such as personal care, home health care, professional nursing, and care coordination. It also covers therapies, medical equipment, and adult day programs. For families, the most common need is a home health aide who helps with bathing, dressing, meals, and getting around safely.
The plan decides how many hours of care it will approve. That decision follows a nursing assessment and a review of your loved one’s daily needs.
How VillageCareMAX home care works in the Bronx
- Independent assessment. First, the New York Independent Assessor sends a registered nurse to complete a Community Health Assessment. A clinician then completes a separate exam.
- Plan enrollment. Next, your loved one chooses VillageCareMAX as their MLTC plan, if they qualify.
- Plan of care. After that, the plan builds a plan of care and sets the approved hours.
- Agency assignment. Finally, the plan sends the case to an in-network home care agency, which matches a caregiver.
This process can take a few weeks. If your loved one is in the hospital, the discharge planner can often help start it.
